Choose below if your student will ride the church bus, will be dropped off and/or picked up or will be riding with one of our volunteers. Be sure to include all information requested. Students will not be allowed to leave with anyone unless all contact information is on file.

For Car Riders, in the box above, please give first & last name of everyone with permission to transport your student(s). Include each person's contact number. Please separate each name with a comma. NOTE: Contact numbers for each driver is mandatory. Students will not be allowed to leave with a driver unless we have their contact info on file.

I hereby grant permission for Aurora Baptist Church to record sounds, images, or video of my child while attending Vacation Bible School. I also give permission for Aurora Baptist Church at its sole discretion, to use these sounds, images, or videos in publications (including print, websites, and social media platforms) owned by Aurora Baptist Church in relation to Vacation Bible School.
